The FIB-4 (Fibrosis-4) Index formula is:
FIB-4 = (Age in years × AST in U/L) ÷ (Platelet count in 10⁹/L × (ALT in U/L)^(1/2))
Where:

  • Age is in years
  • AST (Aspartate Aminotransferase) is in U/L
  • ALT (Alanine Aminotransferase) is in U/L
  • Platelet count is in 10⁹/L
Interpretation Thresholds:
  • <1.30: Low risk of advanced fibrosis (F3-F4)
  • 1.30–2.67: Indeterminate risk (further testing recommended)
  • >2.67: High risk of advanced fibrosis (consider additional liver assessment, e.g., FibroScan or biopsy)
