STUDENT SUCCESS PLAN: LESSON COMPLETION

STATEMENT TO PARENT/GUARDIAN AND STUDENT
On DATE, STUDENTS’ NAME accumulated more than 16 days behind in lesson completions. That is three (3) or more weeks of expected work. Our teachers want to help with your student’s success at Indiana Connections Academy. This success plan will be used to chart the progress of their improvement and as a tool to determine continued enrollment in our school.  
This success plan will run for the next two (2) weeks (DATES). If the plan is followed, STUDENT’S NAME will move back to normal interventions and support. If the plan is not followed, STUDENT’S NAME will be referred to the Engagement Committee. 

GOALS FOR THE STUDENT:

  1. Complete 1 overdue lesson in each core class (Math, Language Arts, Science, Social Studies) each school day.
  2. Once a routine has been established, complete 2 additional lessons per week towards getting caught up. 
Failure to adhere to the parameters of our school’s Engagement Policy will result in disciplinary action, including, and up to, withdrawal from our school. This plan will be documented and could be used in other conferences or engagement meetings if applicable.
