Active cancer (+1) ***
Bedridden recently >3 days or Major surgery within 12 weeks (+1) ***
Calf swelling >3cm (+1)
Collateral supervision veins present (+1)
Entire leg swelling (+1) ***
Localized tenderness deep venous system (+1) ***
Pitting edema confined to symptomatic leg (+1) ***
Paralysis, paresis, or recent plaster immobilization of the lower extremity (+1) ***
Previously documented DVT (+1) ***
Alternative diagnosis to DVT as likely or more (-2)

0 = DVT unlikely
1-2 = moderate risk (17%)
3 or higher = DVT likely (17-53%) -- shoulder receive a diagnostic US
