Investigator Carey, identified that respondent, {=`ee name`}, [Job Title], has taken X bereavement days, totaling X number of hours between Start Date and End Date. Due to the associate hitting the guardrails of excessive bereavement, an investigation was completed. It is confirmed and substantiated that {=`ee name`} violated Amazon's Bereavement Policy [intentionally/unintentionally] by submitting a bereavement request with [Fraudulent Documentation/Relationship/No Response etc.]
* Centralized Bereavement has confirmed that {=`ee name`} was aware of the policy when taking the bereavement leaves.
* Centralized Bereavement has confirmed that {=`ee name`} did/did not attempt to contact local PXT or MHLS about needing time off. 
* Centralized Bereavement has confirmed that {=`ee name`} did/did not have any LOA issues during that time period.
* If AA had LOA issues, state what the issues were and why they aren’t a factor in your recommendation to terminate.
