Due to the associate hitting the guardrails of excessive bereavement, an investigation was completed. It is confirmed and substantiated that {=`ee name`} violated Amazon's Bereavement Policy [intentionally/unintentionally] by submitting a bereavement request with [Fraudulent Documentation/Relationship/No Response etc.]

* Centralized Bereavement has confirmed that {=`ee name`} did/did not attempt to contact local PXT or MHLS about needing time off.
