PT/OT eval and treat 5x/week with LCD on {formtext: name=day} and an anticipated D/C date on {formtext: name=day 2}. See participant for individual sessions for 45-60 min based on tolerance. Please focus on ADL toileting, toileting transfers, bed to/from chair transfers, sit to stand transfers, and bed mobility.
