SAR Justification:
Participant is currently below functional baseline due to recent hospitalization secondary to {formtext: name=diagnosis}. Per PT/OT hospital records, participant presents with generalized body weakness and decreased functional mobility skills (please see attached PT/OT hospital notes below). Participant was previously {formtext: name=previous functional level}. Participant will benefit from SAR placement with PT/OT services 5x/week with a tentative D/C date on {formtext: name=date} to achieve highest practical functional level for a safe D/C home. Rehab will continue to follow up participant in SAR.
