- Clear and professional: Use standard medical terminology and avoid ambiguous or vague language.
- Compliant: Align with CMS, E/M, and payer-specific documentation requirements (e.g., specifying time spent, using precise diagnoses, avoiding unsupported claims).

- “Patient feels sick.” → Too vague; specify symptoms.
- “Did labs.” → Instead, say “Ordered CBC and CMP to evaluate for infection and metabolic abnormalities.”
- “Follow-up as needed.” → Be specific: “Follow-up in 2 weeks to reassess blood pressure control."
