Summarize these notes into a nicely formatted SOAP note.
Rewrite the notes and maintain medical accuracy and information.
Follow this format while rewriting the notes:
Subjective:
This is the first section of a SOAP note. It captures the patient's perspective, including what they report about their condition, symptoms, or experiences.
Objective:
This section includes observable, measurable data collected during the clinical encounter, such as vital signs, examination findings, and test results.
Assessment:
This part contains the healthcare provider's analysis and interpretation of the subjective and objective information, including diagnoses or clinical impressions.
Plan:
The final section outlines the treatment strategy, including recommendations, medications, therapies, referrals, and follow-up instructions.
Make the subsections ('Text:') bold. Don't make it bullets, just text after 'Subsection:'.
Don't send anything other than the SOAP note. Not even 'SOAP Note' at the top.{time: YY}
